HCDI Lead Indexer
Chapter 13. Issuing reports and report status
Source note
Source basis - S3, physical PDF page 10.
HCDI platform rule (Unit 3.11). On a two-party engagement, record the recipient role, a role and never a name, before the Full view. Before issuing, two statements are attested, and each is your personal recorded statement. Each edition's Findings report issues before its Full report. Generate once, since a duplicate leaves two version 1 documents. Never type a sample credential number in the credential line, because every number is verified at issuance. The document is never amended. HCDI Compliance proposes a status change and a different Internal Reviewer confirms it, and every status needs a reason. Common Error. Attesting a statement because the checkbox is there. An attestation is your word.
Issuance creates a fixed record
Basis - general professional knowledge and editorial explanation. External references are identified explicitly; this section adds no HCDI rule.
An issued document and its current status are different records. Generate once and preserve the applicable edition and recipient role. A personal attestation requires knowledge, not routine clicking. A credential number cannot be invented to make an issuance line look complete.

Figure 3.13 Issuance creates a fixed record. Original conceptual illustration; arrows organise the discussion and do not establish a validated causal effect or a scoring procedure.
Original exercise L3 E13
Basis - original fictional exercise. The organisation, events and any numbers are invented for learning. No HCDI result is assigned.
A team generates duplicate version 1 reports and proposes editing one Word copy to remove an error.
| Analytical task | Learner response required |
| Observation | Identify what the stated record directly establishes. |
| Inference | State the interpretation that still needs support. |
| Alternative | Identify a plausible alternative or missing context. |
| Authority | Identify the relevant credential or role boundary. |
Question - What must remain distinct?
Worked answer: Part G, exercise L3 E13.
